Non compliance citations for pain assessment and reassessments continue to raise especially the documentation of reassessments after intervention (PC.01.02.07 EP.3). The lack of an initial comprehensive pain assessment is also frequently cited (PC.01.02.07 EP.1), but organizations can typically show 90% compliance for emergency department patients and inpatients. Compliance issues are less clear for the general ambulatory care patient.
The model policy addresses common vulnerabilities by establishing common sense approaches to this complex issue. It focuses the patient populations requiring a comprehensive assessment and it allows for flexible documentation of pain reassessments.
The policy requires a pain reassessment every 12 hours for inpatients which supports hospitals with a mixture of shift lengths. By performing pain reassessments, for example, on each shift of both 8-hour and 12-hours, staff will comply with policy without raising a standard of care issue.
